Terms and Conditions

Please read the below Terms and Conditions:

By using our Online Referral Service, you are agreeing to be bound by the following terms and conditions.

  1. The fee for a single repeat referral is $35.00, payable when you are contacted regarding referral collection or delivery.
  2. Please allow up to 3-5 business days for your request to be reviewed and processed.
  3. To be eligible for an online referral, you must have had a face-to-face consultation with Goyder’s Line Medical within the past 12 months and have previously consulted the same specialist for the same medical condition.
  4. This service is strictly for repeat referrals only. New referrals or changes to your specialist care require a consultation.
  5. The service is only available during clinic operating hours (Monday – Friday, 8:30am – 5:30pm).
  6. Referral request submissions through our website may not always be fully secure. For more information, please refer to our Privacy Policy.
  7. Patients are responsible for ensuring that all personal and specialist details provided in the online form are accurate and up to date. Incomplete or incorrect information may result in delays or refusal of your request.
  8. All requests are reviewed by a doctor to determine whether a repeat referral is clinically appropriate. Approval is at the doctor’s discretion.
  9. If a consultation is required before a referral can be issued, you will be contacted to arrange an appointment. Standard consultation fees may apply.
  10. Only one referral can be requested per online submission. For additional referrals, please submit a new request.
  11. A repeat referral request does not guarantee that a referral will be issued.
  12. You will be notified by our staff via SMS or telephone once your referral has been approved and is ready for collection or delivery.
  13. Goyder’s Line Medical is not liable for any delays, loss, or failure of electronic communications or postal delivery once the referral has been dispatched.
  14. We shall not be held responsible for any conflicts, errors, or negative outcomes resulting from the use or misuse of this service.
  15. By communicating electronically with Goyder’s Line Medical, you release the Practice from all claims, losses, expenses, and liabilities arising from the risks associated with online communication.
